can I make an appointment?

How may I reschedule or cancel my appointment?
To reschedule or to cancel an appointment please call 405.840.5100.

Who
may I speak to for questions about my bill?
For questions about billing you may call our office and ask for our
billing department.

What
should I do following surgery?
After your surgery you should follow your discharge instructions and
call the clinic if you have questions and/or concerns.
Records
How
do I request a copy of my medical records?
Please call our office at 840-5100 Ext. 8 to request your medical records.
How can I find out my test results?
To find out about your test results a follow up appointment is made
where the doctor will go over the results and discuss them with you.
Insurance
Do
you cover Medicare?
We do cover Medicare.
Emergency
After Hours
If
I have an emergency after office hours who can I call?
If you have an emergency and it is after hours you may call our clinic
at 405.840.5100 and you will be given an alternate emergency number
or you may go to the emergency room.
Prescription
Refills
How
can I receive a refill on my prescription?
Refill requests should be faxed to our office at 405-840-5102 from your pharmacy 2-3 days before they are due. Please take note of the following when requesting refills:
- Narcotic medications will not be refilled if you have not been seen by your physician in the last 90 days.
- Schedule II narcotic refills require a monthly office visit with your physician and must be picked up in our office.
- Medications will not be refilled if you miss a scheduled appointment.
- Any changes to your current prescription will require a clinic visit with your physician.
- We are not responsible for lost or stolen medications and prescriptions.
- Medications will not be filled on the weekends.
- All refill requests received after 11:00 am on Friday will not be refilled until the following Monday.
- Due to HIPPA regulations, we are not allowed to discuss any medications or health information with family members, only patients.
